When Millie is walking through the streets repeating all the NPC catchphrases, she repeats Guy's one too. This implies that she has seen/interacted with these NPCs many times before. Although she knows Guy's catchphrase this well, it only means that she has seen other NPCs like him before who use the same catchphrase. But Guy only sees her for the first time in this scene.

Why did Keeys and Mouser load into the game and chase Guy? As devs both Keeys and Mouser have the ability to load pretty much anywhere in the game at any time they want instead of having to chase him. This also includes the ability to disable Guys' shoes.
