- Gaurav Arts, the film's distributor in India, had cut some of the most violent shots in the film in 1985 in order to receive an 'Adults Only' rating from India's censors board. Among the scenes trimmed were Rambo being tortured in the leech-infested water, Rambo being tortured through electrocution, Rambo's attack on both Podovsky and Vinh's men near the end etc. The cuts were finally restored in 1993 when the film was submitted to be re-rated by India's censors board.
- Some television versions feature the following added scenes: 1. A longer scene of Rambo parachuting to the ground. 2. The pirate boat waving a flag to signal another boat. 3. An alternate take of Rambo spitting on the ground instead of saying the F-word at Podovsky. 4. After Rambo agrees to make the radio call, there's an added scene of Podovsky repeating his directions to Rambo of what to say on the radio.
- In Germany, while initially released uncut in theaters with a "not under 16" rating, most German video releases prior to 2011 are later slightly cut by 40 seconds to reduce violence in a couple of scenes. Despite earning the "not under 18" rating, it still got indexed anyway. Only in 2011 was the movie removed from the index list and restored its FSK-18 rating.
- In a television version there is footage of Rambo parachuting into the jungle after he cuts himself loose from the airplane. It was cut for the theatrical version and cannot be found on any of the early DVD releases. This scene is visible in the current DVD release.
