- Kain and Raziel fight off their various foes, both human and immortal, with their own respective Soul Reaver weapons; Kain attacks with the sword itself and Raziel fights with the spectral Reaver blade, both of whom strike at enemies with lightning-quick slash combos until they are properly weakened (this is shown by a large splattering of blood). Said combos are fast and intensely violent. Both weapons also have a proper impaling finishing move that is unique to the character; Kain's blade will drink the blood of his defeated foes and Raziel's devours their souls.
- As a vampire, Kain must drink blood to stay alive and does so by way of feeding off his stunned, defeated enemies, either by grabbing them and sinking his fangs into their throats or telekinetically extracting blood from their stunned form. Both of these are graphic in approach.
- Concurrently, Raziel must devour souls to stay alive and has two similar feeding methods (one up close and one from a distance), though neither are bloody.

- As with other Legacy of Kain titles, the game takes on a dark/gothic horror approach with the main characters being vampires, demons, necromancers and other assorted terrors.

- At the cusp of Kain and Raziel's confrontation in Avernus Cathedral, Raziel violently rips the Heart of Darkness from Kain's chest. This is a pretty graphic scene.
