After the card game breaks up, Kanger looks down to his lieutenant, who is standing in front of a double door. The right door is open. Kanger comes down and the two talk. The scene changes to the inside of the building before the men enter and both doors are closed.
Saunders, standing behind Littlejohn, is watching the men play poker. Littlejohn picks up a bill with blood on it, comments to Kanger about it, and Saunders has disappeared.
Just before the captain/doctor comes to get in his jeep, the boom mic is very obviously reflected in the windshield, moving around as Kanger moves.
When Saunders asks the lieutenant where he's from, he says St. Paul, Minnesota which he then refers to as "The Gateway to the West". That should have made Saunders suspicious since The Gateway to the West is the nickname of St. Louis, Missouri.
