The season finale didn't have any big cliffhangers for the characters but it still ended with a very big event in which a mentally unstable man shoots his family but had no bullets for himself and all ER is trying to save them. It is as usually very big and fast paced showing the action of the emergency.

There are some small stories for the main characters which I guess will now be more developed in the fifth season. Mainly Dr. Corday trying to stay in the hospital, Dr. Weaver trying to get Morgenstein's position and the percocet being missing.

I really like Dr. weaver, can really feel her frustration especially with Doug just getting away like this.

The end of season 4...

It was the epitome of fan service writing. Dr. Ross acted in the most inconsiderate and unprofessional way, yet gets to be the hero and star of that season finale. It was cringeworthy. Imagine any other ER characters pulling that stunt? It reminded me of when Izzie tried to save Dennis on Grey's. Which got her fired on the spot, and rehabilitated later. But in ER's case, it seemed that the Clooney craze was so high, they turned it into a successful case study, after the slacker struggled with his presentation, earlier this season. Weaver's freak out was hilarious, but understandable. She's one of the few respecting policy, and always gets blamed for it by that Anspaugh guy, who should've stuck with investing in real estate. Carter is an insufferable dbag, and I'm glad Del Amico never reciprocated to his puppy love. I never get into the Peter and Elizabeth relationship either, and I'm shocked Peter pulled the race card on her this time. I always thought he was above all of this... I can't wait for Jeanie to leave too, she's a mess. And Mark is just annoying dancing around problems all the time. Season 4 is an average year for the show. I mostly enjoy the first half, and some of the episodes afterwards, including the two hours season finale. That is not even broadcasted as such. But like I said on my other comments, writers made it really hard for me to root or care for some of the leads, who should be perfect in what has become of Grey's, for acting like irresponsable fools, doing things according to their guts, and ego, regardless of their patients well being.
