As the film crew interviews Wilson regarding House, Wilson says, "Personally, I believe he was just tapping his foot and reaching for the toilet paper." This is a subtle reference to the June 2007 Larry Craig political scandal, when the senator was arrested for lewd contact in a men's restroom in the Minneapolis-St. Paul International Airport. (The foot tap signals a person's wish to engage in sex.) This also happens in the season 4 finale (S04E16)--Thirteen is crying in a bathroom stall, when House's foot slides under the divider and touches hers, making her jump.
House tells the film crew that movement looks good because it provides the impression that the story is also moving forward. This is both a truism in dramatic staging, and a self-deprecating jab at the show's extensive use of this device.
House's mocking stage direction "A little faster. More energy!" is a dig at George Lucas. During the filming of Star Wars: Episode IV - A New Hope (1977), all that Lucas ever told his actors in the way of direction was to act "faster and more intense".
The plastic surgeon says, "Tell me what you don't like about yourself," which refers to the show Nip/Tuck, where Drs. McNamara and Troy frequently ask their patients this question during their consults.
