- When it is enabled, there is a lot of nudity in the game, a lot of it being unexpected. Characters have exposed breasts, buttocks, and genitalia showing, and on character creation you can customize your genitalia.
- However, despite it being frequent, the nudity is completely optional and can be disabled or enabled by the player at their own free will.
- Full-frontal graphic and sexual nudity are present without mods.
Violence & Gore

- Violence is frequent in this game. In combat, blood spills are seen when characters take damage and large pools of blood generate around defeated characters.
- An early scene features several writhing, dying Hyenas, which you must kill, or fight the newborn Gnolls that soon burst from within.
Profanity

- Uses of words such as "Fuck" and "Shit" appear in dialogue. However, as the game's scripting is extremely long anyway, it makes the words less frequent than you may think.

- The game's primary conflict is that your character has an alien tadpole in his or her brain. The game is full of horrifying depictions of torture, body horror, entire areas made of organic flesh, living brains, zombies, and demonic cults, among other things.
- If this were a movie, it would've been rated R for graphic sexuality/nudity and horror violence throughout, and for language.
