- A heavy petting scene, turned bloody.
- Two instances of female nudity shown from the back (the sides of their breasts and nipples can be seen) while they're showering.
- Women in very revealing swimsuits can be seen around a bartender who's sitting in a blow-up pool in one episode. It is also mentioned that these women are prostitutes from the second floor of the bar.

Violence & Gore

- Some strong violence is present, especially in the last three episodes.
- A man is tortured by having his arms and legs cuffed around a metal barrel, with his torturer dropping hot coals into it. It's later revealed that the place burned down with him still inside it.
- A wounded soldier is promised to be taken to the hospital in exchange for information, however when he gives the information he is quickly shot dead. Bound to upset some viewers.
- A man and a woman are about to have sex in the middle of a war zone, only for the woman to shoot needles from her belt buckle and finish the man off with numerous punches to the head that eventually transform his head into a pile of human mulch.
- In several flashbacks to Vietnam, a bunch of U.S. soldiers standing in a destroyed village terrorize a little girl with the intent of raping her, but one of the soldiers intervenes and saves her.
- A group of U.S. soldiers are searching for Roberta in the jungle, and Roberta hunts them while they hunt her. The ensuing chaos is brutal and intense, with spikes and snipers dealing the most damage/deaths.
- Episode 3 is the most violent, with guerrilla warfare between the Russian mob, the Lagoon Company+a few allies, a group of bounty hunters, U.S. soldiers, and Roberta herself all clashing in the streets of Roanapur. People are blasted apart in graphic detail by anti-material rifles and explosive charges; other graphic deaths include chainsaws, knives, pistols, grenades, wires, bazookas, and snipers--all of which lasts for most of this episode and a good portion of Episode 4, and is insanely violent. The series doesn't let up on the brutality--in fact, this episode may even put the Bloodsport Fairytale episodes in the 2nd Season to shame with how bloody, disturbing, and violent it is. It should also be noted that the graphic nature of the violence in this OVA is even more intense than anything that was in the show.

Frightening & Intense Scenes

- The OVA is definitely more intense than the majority of the show, and it is very bleak and depressing, and is bound to upset some viewers with its dark subject matter and themes that include rape, revenge, and the darkness of the world the characters reside in.
- Throughout the OVA, Roberta goes on a downward spiral into insanity, popping pills and drinking nonstop until she starts hallucinating and killing everyone without distinction, nearly killing her own young master at one point. This is done in a rather dark and disturbing way.
- A character has a flashback that involves being beaten and raped by police officers.

Sex & Nudity
- One of the main characters has a flashback where she is beaten and raped in a New York police station (we see a bottle breaking over her head, and an officer pin her to the ground and pull her panties down, but no actual nudity is seen).
- Roberta and a man heavily kiss. She says (after killing the man's soldiers) that the smell of blood turns her on. He positions his gun over her breasts and uses it to feel on them. Holding a gun to her head, he tells her to take off her clothes so they can have sex. She uses the mini guns in her belt buckle to shoot him in the stomach. The sex never happens.
Violence & Gore
- An explosion at a political rally causes numerous deaths, including that of the father of a small boy. The family maid then goes insane and goes on a killing spree, waging war on the United States.
- By the end of it all, Roberta is left extremely crippled, with lost limbs and new disfigurations, and is now in a wheelchair, which may upset many viewers.
