- Meredith has mad motherly instincts as she senses that Zola is awake in the morning and Derek confirms she's right. Callie is nervous about speaking a the TED conference and Arizona helps her picture the audience naked by, well, getting naked. Cristina and Hunt are done having their morning sex when they rush to the hospital after hearing about a gas tanker accident.
At the hospital, Alex interrupts Jo's make out session with Dr. Myers. Jo comes out and checks in with Casey, whose son Parker is sick. His fever is high and he now has a rash. Jo says it isn't that unusual. Casey has been doing some online research and just "feels" like her son doesn't have a virus. Jo says she'll check it out.
Hunt talks to Derek about not just using Shane as his intern.
The victims from the gas tanker crash are starting to come in and the doctors are immediately hit with fumes. Hunt warns that there is to be no smoking or flame anywhere, and no cauterizing.
The first woman wheeled in is 42-year-old Rachel Dawson, who is asking about her son, Ethan. Another man is wheeled in, it's her husband Paul. He's also asking about Ethan, 11, who is in the next ambulance and is asking about his parents.
Jo comes and tells Casey that her son tested positive for strep throat. She's relieved and goes home with a prescription for omoxacillin.
Bailey is checking out a testy victim from the crash who wants to go outside for a smoke. Meanwhile, Ethan is checked out and wants to see his parents. Hunt takes him to see his dad, who needs a CT scan quickly. Paul is wheeled away, assuring Ethan that he's going to be fine. Hunt takes Ethan to see his mom, but the board says she's been taken to radiology.
There's an explosion and Hunt immediately assures Ethan that everything is OK. We next see the guy who wanted a smoke outside the ER doors engulfed in flames.
Jackson takes the "human fireball" for treatment.
April gets a patient named Elise who asks her to focus on doing all she can to save her baby girl. April says she'll try to save them both.
Derek and Shane check Rachel's scans and she has a hemmhorage in her brain. Heather walks in and thinks it's "cool." She tells Derek that Hunt said she needs "to learn brainy things," so she's with them today.
April is working on Elise and her unborn baby. Alex comes over to help and tries to adjust the umbilical cord. Alex wheels Elise away and April turns around to see that her boyfriend Matthew is in the E.R. as a patient. His partner explains that he ran into the crash to shield a kid from the blast with his own body. His back and leg are messed up. He tells April he'll be fine and Jackson takes a look.
Callie gets to the hospital hoping there's no surgical work, but it's clear there will be. Her flight leaves in four hours.
Cristina is working on Paul and she asks Hunt for another hour so she'll know more.
In surgery, Heather asks Derek where he got "it's a beautiful day to save lives." He's a little annoyed. Shane whispers to ask her what she's doing and Heather says Derek makes her nervous. She just wants him to like her. Shane drops an instrument and Heather catches it. Derek praises her "fast hands."
Myers gets Elise's baby out, and Alex is annoyed that he hadn't thanked him for catching the situation in time. Alex takes the baby away after Myers tells him he's "being very unprofessional." Meanwhile, Elise is losing too much blood.
Leah tells Hunt that Social Services has come for Ethan, who isn't happy about it. Hunt asks if he can keep Ethan in a room for the night. Ethan likes it.
Alex tells Stephanie he doesn't know what Jo sees in Myers, but Stephanie goes on to list his many attributes. The father of Elise's baby comes and sees her. Myers hands the baby to the father, and Stephanie then adds that Myers is sensitive.
Casey is back and says her son's fever is getting worse. Jo tells her to calm down, which upsets Casey. This is the third hospital she's been to this week, and she's determined not to get pushed around this time.
Derek gives Shane instructions on what to do next with the patient, and has some fun by tossing things over his shoulder to Heather, who catches them all. Shane wants in on the action, so he tells Derek he's "open" and Derek tosses a cell phone to Shane, who drops it. It was Heather's phone.
Jackson is treating Matthew's ass, which makes April uncomfortable when she walks into the room.
Jo has brought a psychologist over to talk to Casey, who gets upset by this. She insists she's not crazy, her son is sick. Meredith sees this from afar. Casey continues to list all the things her son normally does and just aren't right. Meredith comes over and tells Casey that she believes her and tells Jo to admit her son.
The social worker has lost Ethan, and Hunt goes to look for him. We next see Ethan walking around looking in the rooms, probably for his parents. He sees his mother's in a bed and runs to her, but she doesn't recognize him and starts shouting, "Who is this?" Ethan is stunned, and Hunt walks in just as the scene is unfolding.
Hunt has a late-night snack with Ethan and explains that his mom had a serious head injury, which could be causing her to act "really weird" for now. Hunt asks Ethan to stay with the social worker.
Derek, Shane and Heather look at Rachel's scans and Shane gives Derek a recommendation, which Derek thinks is right. Heather makes one, too, but Derek blows it off. Then he tosses a pen at her and she catches it. Shane's a little jealous of this, but Heather reminds him that he's Derek's "star pupil" and she's just a "circus act."
Meredith tells Casey that her son's scans are completely normal, but Casey runs through her many theories about what the problem could be and Meredith lets her. Casey mentions an article she read about Kawasaki disease, which can cause a heart attack. Meredith says it's a good one that would explain the fever and rash, but there would be other symptoms such as strawberry tongue, peeling skin and red eyes and no positive strep. Meredith says she thinks it's strep. Casey apologizes to Meredith, knowing she's a surgeon who doesn't normally handle these kinds of case, but Meredith says she owns the hospital and can do whatever she wants and she wants to do some more tests.
April walks in on Elise's husband talking to his wife in her sleep. She doesn't want to interrupt. She walks in and he tells her that Elise likes the name Rose for the baby. April thinks it's beautiful. He kisses Elise on the head and goes to see the baby. Jackson comes in and April shares the couple's story they had dated in college but she left, then years later they both separately moved to Seattle and joined the same bowling league. She wonders if they were being pulled back together. Jackson says the same fate put them on the freeway next to a gas tanker that crashed.
Callie and Webber come to Bailey and ask her to map their genomes. She says her research is not a toy. Arizona is surprised to see Callie still there, and says she's sorry she'll miss her speech. But she notes that now Callie will be able to "spice" her speech up for next year. Callie takes some offense and Arizona stammers to cover herself.
Cristina is exhausted and gets a page about Paul. At the same time, April goes to Elise who is crashing. Cristina gets Paul's pulse back. April isn't as fortunate with Elise. She's gone.
Jackson finds April, who doesn't know why this one is hitting her so hard. She says it's "not right" and she can't tell Elise's husband. Jackson offers to do it, but April says she'll do it. She cries and puts her head on Jackson's shoulder. He puts an arm around her and it looks like they might kiss but Alex comes in for an update on Elise. Jackson tells him she didn't make it and they were about to go tell the husband. Alex takes the moment in and says Myers' name is on the file, calling him "Peckwell." He asks if he should make Myers do it, and Jackson and April both agree.
Meredith talks to Cristina about Casey's son while they ice him for therapeutic hypothermia. Hunt doesn't like this idea and asks if Dr. Russell agreed to it.
In surgery, Derek lets Shane in to open up part of the skull. He then asks Heather to step up and do the next part. Derek is impressed with her "fast hands."
Meredith tells Casey she can't find anything wrong with Parker other than the strep. Casey says the feeling that there is something wrong with her son won't go away. She asks Meredith, "How do I ignore this?" She asks how she can ignore her gut telling her there is something wrong. Meredith says nothing.
Alex finds Myers, again interrupting him making out with Jo, and tells him that Elise didn't make it and says the news should come from him. Myers says she's not his responsibility, and Alex calls him a douche. Alex says he'll go do Myers' job again.
Meredith calls Jo over to run through Parker's test results again. Jo says the labs just updated and the strep culture gave a false positive. It's actually negative. Meredith realizes they're "already at Day 10," and she runs to check Parker's eyes. She checks Parker's eyes and sees they're very red, and tells Jo to prep treatment for Kawasaki disease. If they start immediately, they can avoid permanent heart damage.
A shaken April watches through the nursery window as Alex breaks the news to Elise's husband. She sees him cry and goes into the room to put her hand on his shoulder.
Casey cries, too, as she looks through the window to her son's room. Meredith comes and shows her scans of the fluid around his heart. She says the IV will fix it and he's going to need another echogram in six months. She tells Casey she was right about Kawasaki disease and says Casey saved her son's life by not giving up.
Derek tells Shane that Heather is going to be on his service for the rest of the week. He says Hunt was right about interns needing to work in different areas.
Arizona brings Callie to a meeting room and there's a video production set up. Webber made some calls and arranged for Callie to give her TED speech on live video. Callie is nervous, but sits in the chair and is live just seconds later. She's introduced and Arizona urges her to just be herself. Callie starts her speech, which is supposed to be about cartilage, then puts down her notes and starts talking about the terrible she's had with the plane crash and nearly losing Arizona and losing the father of her child. She says she's an orthopedic surgeon by trade, but she works with cartilage, so she's spent a lot of time thinking about "what holds us together when things fall apart." Arizona looks pleasantly surprised.
April goes to see Matthew and asks him if he believes that God plans for two people to be together. She tells him about the patient who died and left her husband with a new baby. She says it doesn't make sense. Matthew tells April that he gets it. He insists he's fine and isn't going anywhere and he loves her, too. April looks a bit surprised.
Hunt tells Cristina that Russell agrees with her approach. Cristina asks Hunt what's going on, why he's questioning her decisions. He says the man has a young son and he just wants to make sure he sees his kid again. Cristina insists she wouldn't only do what's best for the patient and that she, too, wants him to see his son again. Hunt hugs her and apologizes, saying it's been a long day. Cristina says they should go home, but Hunt says he has to wrap up a few things first.
At the bar, Shane finds Heather and tells her he knows what she's doing and he insists he's not going down without a fight.
Back at the hospital, Hunt brings Ethan to see his mom, who is sleeping. Hunt tells Ethan that both of his parents have a lot of good doctors taking care of them.
Casey sits in her son's bed and he's made a drawing for her. Meredith comes and sees Casey smiling, and gives her a smile of her own.
Meredith then comes to Bailey and asks her to map Zola, because she doesn't know about her background. And she wants Bailey to map her, too, because she wants to know if she's going to get Alzheimer's. Bailey says nothing, but nods.
