- Extreme sexual content and full frontal graphic nudity.
- There is a scene where you see the player's full body nude for a few minutes.
- The player gets sexually assaulted by some of the enemies in the game.
- Naked dead bodies with exposed genitals.
- A man can be seen masturbating to decapitated and deceased bodies. While this is missable, you likely won't miss it.
- The cannibal that wants to kill you is in his underwear, and covered with blood.
- The twins from the first game appear in the DLC. (They are both fully naked)
Violence & Gore

- During the initial stages of the breakout, you can briefly see a security guard covered in blood tackling an inmate. After some struggle, he manages to strangle the inmate to death.
- The game is filled with mutilated corpses, gore and blood from start to finish.
- Eddie Gluskin is a serial killer that is seeking for a "perfect wife." This means that he captures male patients and tries to turn them into a female in a very gory and crazy way. He cuts off the genitals (not seen; Gluskin in standing in the view) of an inmate who screams aloud, flailing and bleeding heavily. Shortly after, the inmate dies due to suffered trauma. Another one has his stomach cut open and genitals chopped off. Gluskin then slices his head with buzz saw. A highly gruesome and disturbing scene. Even those with strong stomach will likely make to squirm.
- Later on you get caught by Gluskin and you see yourself in a first-person view trapped. During this scene, you can partially see your character's genitals. But before the buzz saw cuts it off, you are saved by an inmate. Extremely intense.
- In one part, you run away from Eddie Gluskin and you have to jump onto a ladder that is far in reach. You grab on the ladder, but the ladder breaks in you end up falling down at the bottom of the elevator. You end up getting a giant splinter on your ankle, and blood spurts out while you take it out.
- The Walrider escapes, and causes chaos in the asylum. Some inmates and characters that get killed by the Walrider are shown, and when they are, blood erupts from their body.
- The cannibal is shown eating a body. Blood is shown, and bone crunching sounds can be heard.

- Many uses of "fuck," "shit," "bitch," "bastard," "damn," "hell, "Goddamn," "Jesus Christ," "crap," "cunt," "tits" and "balls."

Frightening & Intense Scenes

- While Eddie Gluskin chases you there is very intense and frightening music which might freak out some players.
- There is scenes of aberrant sex and torture. Very graphic and disturbing.
- This game isn't for weak-hearted. Part when Eddie Gluskin brutally kills two inamtes is one of the most chilling sequences ever seen in video-game history. Even those with strong stomach or very adult will have trouble with it.
- Part that it takes to navigate outside the asylum is quite eerie.
- This DLC is about as terrifying as the original game, but it's got less jumpscares and more of a suspenseful feel to it.

Sex & Nudity
- One of the asylum employees licks the player's face.
- There's a part where an enemy named Eddie Gluskin captures you and tries to turn you into his wife. This is more disturbing than explicit because the next area you go into has hanging naked bodies in the room.
Violence & Gore
- Near the begining your character is thrown on the ground and punched after he sends an email to Miles Upshur asking for help.
- A man walks into a room when you try and use a radio and destroys the radio. The man then strangles you with a baton - but before he can kill you, he hears Chris Walker and runs off, telling you, "Do me a favour and die here, Park!"
- Eddie Gluskin dies by being graphically impaled on steel rod.
- Just before the end, Jeremy Blaire is wounded and asks for help. When you go close to him, he stabs your stomach with a knife and says, "F*cking die already!" But before Jeremy can make the final blow, the Walrider appears and seizes Blaire. After some struggle, it enters Jeremy's body and causes him to explode from the inside, leaving his blood on your body.

- Eddie Gluskin drugs the player with a sleeping gas which lasts for 12 hours.
Frightening & Intense Scenes
- There is a moment where you will have to pull a lever to turn off electricity (that is blocking the door you have to pass) when you turn around, an inmate is standing very close to you (he wasn't there before), with the killing intent. Very sudden.
