Sex & Nudity

  • Naked dolls around.
  • A boss fight with a female character involves her being grossly mutated and having a hive of insects that looks to be in between her legs. It's also a weak spot that players can shoot.

Violence & Gore

  • The player can have their hand and/or foot cut off with a chainsaw. The player then retrieves their severed body part as an item.
  • This game is extremely brutal and gory but less disturbing than a game like Outlast.
  • Lots of violence. This game is one of the most brutal "Resident Evil'' games ever made.
  • Whenever a humanoid enemy is shot, stabbed, etc., blood sprays and a wound is seen where they were shot/stabbed/etc.
  • Whenever the player is attacked, blood is seen on whatever weapons the enemy was using.
  • Andre is stabbed through the mouth with a metal bar. It happens offscreen.
  • A man is stabbed through the hand with a knife.
  • You can get attacked by a man with a chainsaw.
  • Ethan is suddenly attacked by his wife Mia with a chainsaw and with a piece of wood, Ethan has no choice but to fight back with an ax, this fight is very bloody and brutal but it should be noted that Ethan is acting in self defense
  • The Baker family is seen eating what seems to be human organs. Jack attempts to force-feed Ethan, but he rejects it, then gets stabbed in the mouth.
  • Zombie like enemies called "the molded" are made out of black goo with very sharp teeth and sharp claws. They can have their heads blown up or they can be dismembered in anyway.
  • A man has a nail impaled in his hand before then having a quill pen stabbed into his chest. He pulls both out slowly and painfully.
  • A man has a message painfully tattooed on his arm by a robot.
  • A man is burned to death in an accident.
  • There are many gory and disgusting things in every place, with what seems to be human remains in fridges and certain places.
  • A man is brutally killed, he vomits black goo, his skin turns grey, and he then dies.

Profanity

  • The words "f**k" and "sh*t" are heard in dialogue.
  • Alongside Revelations 2, this easily has the most swearing out of any game in the series.
  • Several profanities in the Japanese dialogue.
  • Around 75 uses of f*ck in the main story and around 30 other uses in the DLC's. There are uses of c*cksucker, sh*t and numerous other swears throughout the game.
  • LOTS of profanity. Plenty of uses of the F word and other profanities.

Alcohol, Drugs & Smoking

  • Unlabeled bottles can be found around the main house. Presumably alcohol. The player can craft med-kits to heal themselves using herbs and "chem fluid." You can also find steroids throughout the game, which give you a permanent health boost.

Frightening & Intense Scenes

  • The game is especially intense in virtual reality.
  • This is one of if not the most intense Resident Evil games ever made. Previous titles, while scary at times, always had a corny B-Movie feel to them while this one is much darker and violent. At times it can feel like a black comedy of sorts, but it is still intense.
  • The whole game is filled with scary imagery and effective jump scares. Not for the faint of heart.
  • The game is scary, but not as disturbing and terrifying as Outlast.
  • By far the scariest game in the series
  • The Baker Family are extremely scary in terms of personality and actions, though they're also quite darkly comedic.
  • The house is very scary, with lots of decayed and disgusting stuff strewn about.
  • The Molded are very deformed and scary looking, not to mention they're also hard to kill. There's also molded growth on the walls and other places throughout the house.
  • It's strongly implied that at least twenty people were brutally murdered and cannibalised. One victim in particular was burned to death.

Violence & Gore

  • The opening dinner scene is very graphic. The entire dinner table is covered with what seems to be intestines. Jack gets mad at his son Lucas, and amputates his hand, stabs your mouth with a knife, and kicks a chair at his wife. Other than the cop scene, this may be the most graphic scene.
  • A cop has his head chopped in half with a shovel. This is one of the most brutal scenes in the game. It should be noted that certain countries have altered specific scenes, including this one, to make it much less brutal.
  • After the cop gets killed you see the body a few times later either pinned to a wall or decapitated. You also attack mia (ethans wife) with an axe/gun and crowbar in various scenes
  • The Bakers are incredibly violent and psychotic individuals. Something really disgusting and bloody happens everytime they are onscreen.
  • Lucas is the most deranged, psychotic person in the game. In a optional video tape, he forces you to be burned alive, get a message cut into your arm, and more. He also places C-4 in certain areas which blows up the player if triggered. In the DLC's, it goes into much more detail on what he does.
  • Jack punches Ethan before stomping on him. The game makes the player think he just passed out. It's not revealed until the near the end of the sequel, Resident Evil: Village reveals that he actually died and that his entire body was replaced with mold that assimilated him and his memories.
  • Jack makes Ethan drive into several irons bars. Ethan gets out of the car then Jack is burned alive after the car catches on fire. Still surviving this, Jack then shoots himself in the head in front of the Ethan, loads of blood sprays and gory wounds are seen. Jack does this to prove he can't be killed, and any wounds will completely heal.
  • Ethan and Jack get into an intense fight with chainsaws. Jack ends up getting brutally 'killed' before regenerating.
  • Ethan is forced to stick his hand into the decapitated neck stump of a police officer to retrieve a key. His arm is covered in gore when he pulls it out.

Frightening & Intense Scenes

  • Mannequins scattered around the place change in number. These changes are hard to notice but when noticed can be quite unnerving.
  • The son, Lucas Baker, makes you participate in a Saw like game. He forces you to solve a puzzle before you were brutally dismembered, and during the puzzle you are injured and there is plenty of blood shown.
  • In a DLC, Lucas Baker traps you and another person for a game of blackjack, but losing will have you losing your fingers, getting electrocuted and chainsawed in the head.
  • In a cassette tape, three people enter the house you are in, the first guy disappears, they search around the house until finding him with his eyes and mouth full of blood, the camera cuts while you hear screaming and smashing, very intense.
  • Much of the game revolves around a sadistic young girl who kills people for fun and turns them into the molded or sadistic killers, she's extremely violent and scary.
