That guy just run and apparently, he is faster than a car.
The older guy wants to adopt him but, even though he raised him, there is no father-son relation at all.
There is not much dialog between the girl and the boy. We just have to assume they simply are madly in love for no apparent reason.
The older guy dies on the spot when the car hits him even though he saw the car coming and had all the time to move.
We don't get to know more about the other doctor, or his daughter or his other problems, we are just suppose to assume he is a bad guy.
The movie looks like is made by 6 grade amateurs.
